I didn’t actually know this. So is the only reason that James sees Pyramid Head the way he does is because he *assumably* saw the executioner’s design when he traveled to Silent Hill with Mary?

You’re still wrong. Pyramid head was created to punish James’ from his sins and takes on a warped appearance of the executioner that James read about during his honeymoon. So no he’s NOT the actual executioner.
