That is, we generally only require a prescription if the benefits of a drug are already known to come with caveats
I'd say - prescriptions are only required if the drug is known to be effective

Effective in what sense? This seems straightforwardly false. I can't overstate how effective non-prescription drugs have been for me;

likewise how ineffective prescriptions have been (SSRIs come most prominently to mind.)
