Some ketamine links (I'm going to be the person who leaves a pile of links before going on to next project.) I'm not stipulating that esketamine = ketamine, and I'm not using the terms interchangeably.

https://www.ncbi.nlm.nih.gov/pubmed/29446408  https://www.ncbi.nlm.nih.gov/pubmed/29446381  This appears relevant to the rapid effect of ketamine in relief from depressive symptoms, as well as to the need to combine with therapy for sustained remission. If you can't access the FT for some reason, DM me & I'll send.

Aetna is an insurer. Their reviews are cold but thorough. (Remember that their analyses don't include the cost of lost work, engagement w family/friends, despair. Those things cost their shareholders nothing.) http://www.aetna.com/cpb/medical/data/900_999/0938.html …

Ketamine is fast, it's effective a fair amount of the time, it's expensive & time-consuming (not as much as depression though!) 2 visits per week, each taking up a day of pt time & ~ a day of caregiver time, plus ~a half-day of clinic time for 2 & up to 3 wk plus therapy.

Then continuing up to 1x/wk for a month, with ongoing therapy, extending intervals out. Again, opinion alert, therapy is essential as part of treatment. https://ajp.psychiatryonline.org/doi/10.1176/appi.ajp.2017.17020239 … https://www.karger.com/Article/FullText/457960 … https://www.ncbi.nlm.nih.gov/pubmed/30528745

Therapy ends up reducing cost to the pt/system - after induction, ketamine sessions extend further apart if pt involved in therapy, more likely to taper off. (There's no therapy requirement in the possible s-ket REMS. It's focused on monitoring post-administration & diversion.)

r-ketamine is moving towards clinical trials. And... oh. I see. That lab was acquired. OK, that's enough internet for today. Tl;dr: racemic ketamine works, it's not a panacea. Combine it with therapy.

what’s your thinking on therapy? is there evidence that it “works” for something or more of a gentle medicine thing?

therapy is intended to address distorted thoughts/feelings/behaviors during/after induction, d/t +plasticity, you're more receptive/responsive to that therapy. this is a tweet-grade explanation, mind you. the Biol Psychiatry article above is a more in depth discussion

this and the side barred similar articles aren't a bad place to read on increased cell growth, & this one discusses ketamine vs classical psychedelics as targets, as well as going beyond just [vague hand wave] "plasticity"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6082376/ …
