    and antimicrobial resistance () are macro-critical issues as well. Investing in preparedness and prevention will mitigate the damage globally. Adequacy of core systems should be in Article IV consultations.

    Thanks for the review, though it contains a number of errors. For one, the inspiration for the title is fully explained (Ch 4, note 8) It has nothing to do with Clint Eastwood and everything to do with Katherine Anne Porter.

    A year ago, I offered thoughts on the next pandemic, in particular its possible effects on society and the reaction of the government. This review, of Laura Spinney's "Pale Rider," about the Spanish Flu of 1918, seems timely now.

    Actually John Barry’s (very good) book was very much the American story of the . If you want the global story - as far as we know it - you need to read mine, Pale Rider

    This graphic is embarrassingly ill-informed. No idea where or how anyone can have determined a ~1% CFR. Also the R0 for Spanish flu was 1.8 (from john Barry, who wrote the book on S.F.) while latest study from China pegs it at R0 = 4.08 for nCoV.

    I don't believe we have enough data yet for to be able to make a firm conclusion about its transmission and rates. The only way to make sure it does not turn into a global is to strictly control its spread and develop a vaccine for it ASAP.

    Indeed. When the vast majority of cases have not yet died or recovered, dividing deaths by total cases to get that number is meaningless. Recent more careful estimates put case fatality rate at more like ten per cent, similar to SARS.

    Whether or not quarantine will contain the virus is a live question - but it seems to me it definitely won't if exceptions are made

    I don’t get this. Quarantine for some and not others? Why are these expats putting everyone along their route - including potentially themselves - in danger? And why are their govts humouring them?
