You fix it when you can confirm that it's wrong. (If you know it's wrong at the time you don't publish it.)

Sometimes you're positive something is right - good sources say it, it's in a document, etc. - but ultimately it isn't. It happens. Sources, even good ones, make mistakes. Better information comes to light. I don't think you can find a news org that hasn't had this experience.

That's a hard rule to apply. Sometimes sources lie. Sometimes they're just wrong. (Often - at least in our paper - they're quoted by name; we don't do anonymity very often.) PP has published a good account of how it made this mistake; it's worth your time.
