Adding an Oxford comma after Scotland would actually increase the ambiguity! The normal format for a city/place pairing like that is to have the larger place set off with commas on both sides. The lack of a comma subtly (too subtly) indicates Scotland is not paired with Paris.

However, as is pretty much always the case, if the clarity of a sentence rests on a single comma, you probably need to rephrase. “In places like Scotland, Paris and Tbilisi, Georgia” works much better, with or without an Oxford comma.
