I don't do any kind of medical or medicine-adjacent human subjects research, but I'm pretty surprised that separating these isn't standard practice for all patient populations. In any mental health context, it could be about strategically downplaying symptoms that the patient (correctly or erroneously) believes might be grounds for denying them their freedom.

I was selectively downplaying my depression and anxiety symptoms years before I even thought about medically transitioning, for a whole variety of reasons. In hindsight, some of these reasons hold up better than others.

This is not something I know about.
