I suppose I'd see this fundamental characteristic you point out as dysfunctional government - but perhaps there's a better term for it. I don't intend to frame this as purely government problems. But the unintended consequences certainly implicate govt, no?

American voters have very few mechanisms for demanding govt simply do things better. The argument is always to just to not do The Thing at all. Structural problem. CA/feds/whoever can certainly run complicated systems effectively - they just need tools & accountability.
