I've been talking to a bunch of small medical practices this week about how they do billing and claim processing with insurance companies. Interesting quote from one doctor: "With Obamacare and rising deductibles, the doctor's office often becomes a collection agency."

Not really. High deductible plans are the direct result of the ACA. It has put medical practices and ancillary service providers in a difficult position of requiring payment prior to service.

Before those high deductible plans, those people were probably uninsured and not getting care, or getting it at the emergency room

The trade off was to decrease uninsured and increase underinsured. HDP have increased substantially this decade as fewer employers can offer healthcare affordably, which was by design, to switch to exchanges. CBO now estimates net cost for this trade off.
