      I’m taking issue w the twitter pile-on from @sarahkliff’s (well-written) article re SFGH’s ER. It’s SFGH to me bc I trained there, and frankly Zuck is irrelevant to this. I live in SF and practice at a large safety net hospital in Oakland. 1/ https://goo.gl/u9acCh

      First, I will say that abhor the billing practices and cross-subsidization tactics that were referred to in the article. Residents of SF already pay taxes and that should be enough without getting fleeced in the midst of a traumatic injury. However...2/

      This article and following dialogue fail to recognize the primacy and legitimacy of SFGH’s mission first and foremost to caring for the poor and vulnerable. I would argue that this is an intention that may not be served by opening doors to all patients, esp those w insurance. 3/

      I spent thousands of hours providing care at SFGH, including emergency care to women. It was beyond a rarity to see an insured patient. I wonder what proportion of SF’s residents - who are amongst the richest per capita in the country - this issue actually affects. 4/

      SFGH is a sanctuary for the homeless, undocumented, those suffering from addiction, and many others. The ER is it’s living room. The hospital’s design unapologetically reflects its mission. 24/7 interpreters, robust social services, inmate care, abortions for poor women...5/

      ...Public psych care, long-term care, opiate replacement therapy, and for some, a bed and a warm meal... All delivered by some of the nation’s most qualified providers. In doing so, it’s patients - who are mostly of color - know that their problems deserve care w dignity. 6/

      Yet tweets on this article include thinly veiled classist and racist insults to SFGH’s primary patient base. “I’ll bet the addict whose been strung out all day won’t get an ER bill.” Damn right she won’t. That’s the point. This hospital is not meant for you, or for me. 7/

      Because it’s vital to preserve spaces meant *only* for those who aren’t valued elsewhere. While I hope the SF Board of Supervisors reconsiders it’s fee schedule, I honor SFGH’s unwavering purpose to serve the most vulnerable. Even if it means pissing the rest of us off. 8/

          Yes I’m at Highland. To be honest not sure. Would wager we get even fewer insured patients than SFGH given our surrounding communities. I have seen less than a handful in my nearly four years there.
