      This isn't an entirely fair statement actually. @sfbos prob rubber stamps whatever @sf_dph says.       TLDR: SF General's payer mix is more than 80% Medicare/MediCal and private hospitals often don't take govt insurance from low-income patients.       Keep those numbers in mind when reviewing “Medicare for All” proposals: if hospitals served everyone at Medicare rates, without any cross-subsidies from private payers, they’d all go bankrupt. Admistrative costs would go down, but rates would absolutely have to go up.

      I don't think a single urban hospital that has to de facto serve an 80+% low-income population because no private hospitals will, would be representative of a situation where everyone would be in the same pool.

          but ZSFG is still supposed to serve all the city's residents, and not run a perverse lottery. it's still unusual not to negotiate rates with private insurers

          The key point is that docs and hospitals have become critically dependent on higher rates from private payers, contributing to runaway healt care cost inflation.
