      Wait. Doesn’t this violate CA law on user fees? If the City is setting rates extra high so as to recover the costs of the patients who can’t afford to pay, then by definition the fee doesn’t reflect the reasonable cost of providing services?

          I think what I’m remembering is Prop 26 (2010), but there may be associated statutes. Basically, taxes are broadly defined to require voter approval. One of the exceptions is for user fees, but those can’t exceed the reasonable cost of providing the service.

          Of course, you might have to litigate what is “reasonable,” but if the fee is explicitly set to recover costs from those who can’t afford to pay, it would seem to reflect more than the reasonable cost.
