Or how would repeated, emergent behavioral patterns of harassment get moved up to a systemic level within the org from a one-off level. I'm not talking about yes/nos on specific pieces of content. But stuff like we've seen this behavior X times & now we need a new rule/norm

In practice, recognizing the need for new/modifies rules is the easiest part. The tough thing is figuring out how to write a rule that solves the problem while a) controlling for unintended consequences and b) being something that 10k people can do consistently.
