NEW: UK #COVID19 paper from @PHE_uk @MRC_BSU on Delta severity now published Delta associated with increased risk of hospitalisation vs Alpha BUT - when stratified by vaccination status - only unvaccinated had significantly higher risk A closer look🧵 http://www.thelancet.com/journals/laninf/article/PIIS1473-3099(21)00475-8/fulltext …pic.twitter.com/z9JPuOdkNc

🧑‍🤝‍🧑Study included 8,682 Delta cases and 34,656 Alpha cases diagnosed March 29 - May 23 🏥 196 (2.3%) Delta patients and 764 (2.2%) Alpha patients were admitted to hospital within 14 days 💉 Most (74%) cases were unvaccinated; vax rates were similar in both Alpha & Delta casespic.twitter.com/NaMgOOfa5q

The vaccination point is *key* to correct interpretation of this paper During the study period, in England - 1st dose coverage rose from 50% to 70% - 2nd dose coverage rose from 7% to 42% In this study: only 26% of cases had 1st dose and 2% had 2nd dosepic.twitter.com/yG9OJUkBZ4

The small number of vaccinated cases in this study show: 1) The impact of the vaccination programme. Vaccines work💉 and are protective against infections 2) The number of vaccinated cases is too small to reliably assess a difference in hospitalisation (wide CIs)

Results: Adjusted analysis (for age, date, ethnicity, area, IMD, vax status and travel abroad) found - Admission: Hazard Ratio 2.26 (1.32–3.89) - A&E attendance OR admission: HR 1.45 (1.08-1.95)pic.twitter.com/oSLcbtlZvi

Results: When we asked whether vaccination status would reduce or eliminate the excess risk of hospitalisation, we found that it did: - Unvaccinated: HR 2.32 (1.29-4.16) - Vaccinated (at least 1 dose): HR 1.94 (0.47-8.05) Note - small numbers, wide CIspic.twitter.com/aCj5Px7987

This study aimed to assess the severity of Delta variant relative to Alpha. Important to remember!! Vaccines are extremely effective against hospitalisation for BOTH variants: 💉 80% after Dose 1 💉💉 95% after Dose 2 https://assets.publishing.service.gov.uk/government/uploads/system/uploads/attachment_data/file/1013553/Vaccine_surveillance_report_-_week_34.pdf …pic.twitter.com/RlUWlPslNw

This study found two-fold increased risk of hospitalisation for Delta variant It suggests Delta is not only more infectious, but also potentially more virulent. The good news is that immunity through vaccines seems to mitigate any additional harms inherent to the virus

It is possible that the increased risk may be partly explained by confounding due to factors that could not be adjusted for in the model, such as lack of information on co-morbidities.

On the other hand, the risk may have been underestimated due to limited info on reason for hospital visit. Injuries were excluded, but some non-COVID visits may have diluted the association.

Sorry but this is not supported by your analysis. Equal risk increase by delta (ns interaction) … vacc has similar risk reduction. Key for absolute number of people needing hospitalisation. Vacced in absolute terms will need hospital more than with alpha.
