My experience: I've actively asked for opportunities to help me along the promotion path and for feedback. Didn't always get that. Have been promoted within the same company at the companies I worked for in the US. Some promos came because I was started at the wrong level.
Only 1 or maybe 2 came without me directly asking for what gaps I need to personally fill to be performing at the next level. I'm not sure how much I could attribute to privilege, luck, right manager/place/time but I suspect non-zero percentages.
That being said, usually I'm looking to improve mostly and the shortest time I've stayed at a company is 2.5 years (just for data points)
