As a medical biller that manages the full revenue cycle I can say this type of denial is the norm. Sadly healthcare insurance is a game. APPEAL that denial. Reach out to the Surgeon/Doctors to make sure it is noted that this procedure was a Medical Necessity - not elective.

@Aetna minimum once a week to appeal (far less harrowing (sorry to hear!) but equally legitimate) medical procedures and the stupidity of it all is unmatched. plus they make it impossible to follow up with the same person on the same claims. get it the fuck together -
Someone that I know once told me to keep a journal of sorts.. Everytime you talk to someone, get their name, write down the time & date of the call, get an extension # if they have one, & make a note about what the call was about. They fight organized people less. It's bizarre.

Another good reason to nationalize the insurers, use their existing infrastructure to administer a national health system, and then find a fun place for all the executives to hang out for 25 to life. Same with pharmaceuticals.
