So doesn't that practically mean that people can only "cash out" USDT for as long as other people are depositing USD? What do USDT holders do if the sell book dries up?

People can only sell USDT on Kraken if there are buyers willing to give them USD for it. If there are no buyers, you can't sell it. We've seen prices under $0.90 in the past.
