Assuming all patients are paying market rate for care, of course the hospital would serve them. Laws requiring seat belts and helmets assume externalized costs to be absorbed by society. If society is paying, it should be able to reject bills resulting from extreme recklessness.

The willfully unvaccinated who wind up in hospitals from Covid should not receive priority medical care over other very sick or injured people who are as much in urgent need of medical care.

What does "society paying" mean here? (Generally that means paying through taxes.) Does that mean that someone who is unvaccinated would have to pay taxes to contribute to the treatment of covid patients, even though they would not get covered if they're a covid patient?

It could also be increased socialized costs of medical care. Where do the costs of uninsured/uncovered medical expenses go? Hard to measure societal cost of stuff like orphaned kids. Hospitals ideally could auction care and people could take as much risk as they can afford.
