Yeah I remember looking into it with Luca a while back. What's interesting here is that there appear to be a lot more than the last time we found some.
Twitter had “released” usernames for a bunch of inactive accounts last year, I wonder if these are those accounts?

I found 1 in 200k. Never got to the bottom of it.

It's very rare. About 5 times out of 1M accounts. They seem to have started to tweet in the last few years. I don't think it's a legacy thing.
