If this was caused by a person, please don't be too hard on that person. In the past, when things like this happen (in the IT world), we always view it as a systematic issue that may be due to poor procedures / documentation / etc.
These things happen but it's a great ...
... opportunity to review procedures, etc. You guys do amazing work and this was just a hiccup to an otherwise amazing project that benefits society.
