I believe they last assertion is false in some cases. But it's not really them punishing Person A they are just complying with the DMCA. At least to my knowledge.

DMCA does not allow false DMCA / Copyright claims at least to my knowledge, but IANAL (I Am Not A Lawyer) so I could be wrong on this, but that is what I have heard for the past nearly 15 years since I first heard about people having issues with false DMCA/Copyright claims.

How should Person B have been punished? Well, to be quite frank, the exact same way content creators are. 3 strikes and you're out. 3 spurious copyright / dmca claims, lose the right to file any further copyright / dmca claims.
