For the logic to work I think this must also have been happening in every country on earth prior to development of vaccines.

The explanation is that multiple metrics factor into whether someone goes on a vent (I have seen "oxygen saturation" mentioned), and those metrics stack up to whether that person is dying already. Vents are considered life support. They're admittedly not very good at it.

Many health care workers have "palliative care only" directives at the point where a ventilator would otherwise be used. That means you no longer attempt to treat the problem. You want to refuse a vent, you're welcome to it - in fact, I might even recommend it.
