You are assuming that insurance companies want to make it easy to file claims. Auto insurance pay attention because you choose your insurer. Since health insurers are mostly chosen by employers, they have negative incentive to be easy to use by the insured.
yeah this is true idk how the costs work out in the employer/insurer relationship but wonder if at least at the Fun SV Tech level there could be a pressure for employers to pick more user friendly insurers for their employers

Given the political/economic incentive structures here, there’s just very little upside to providing better service. Most of CA is served by literally two insurers—the political:economic obstacles to starting a new insurer are so huge that there’s little competitive pressure

True. The insurers can not make it too horrible, else employees will complain. But few people chose their jobs based on differences in heath insurers so making it things moderately difficult and annoying is optimal for the insurers.
