The prevalence of bad design despite the large influx of designers. Some problems may be systemic in org but I think much is related to quality of design education or people who claim to be UX but are really just doing UI. The two aspects feed off each other.

Related: there is a vast difference between designing for marketing sites/ e-com and designing for tools/SAAS/Enterprise. Being good at one doesnt equate to the being good at the other.

Signifiers need to be interpreted. Affordances are directly perceived and indicate a personal interaction potential with the environment. Signifiers provide value in their meaning, while affordances provide value in helping people know possible actions.
