Not delegating enough. It's hard to do when you built the first version of the product and have a clear vision for the future, which are usually the two best initial conditions for success.

Alternatively: (1) Doesn't buy people in; makes decisions unilaterally or (2) Conflict avoids, delays, looks for consensus.
