LV Aneurysm. I'm curious what else might be in the differential however as I can't think of what else might cause this outside of a rare pericardial tumor as the silhouette so cleanly interfaces with that of the heart.
I mean, cardiac tumors are rare. Myxomas usually grow in the left atrium (this CxR shows a LV deformity), rhabdomyomas are usually common in children (our patient is in their 60s), and cardiac lipomas are rare.
I agree, which is why aneurysm is the only thing that jumps out at me given the very striking appearance of this CXR. LV dilatation/hypertrophy are quite common but the radiographic appearance is garden variety cardiomegaly.
