Great summary. Didn’t know that the NICE guidelines are just based on WBC. The differences in practice are interesting. It’s all about risk (or perceived risk) tolerance. But whose risk tolerance should we be thinking about...? The doctor? The parent?

I think we are (at least I am) probably bad at understanding risk, and even worse at explaining risk to families. Which is... not good, since it’s the cornerstone of much of what we do

First step is knowing baseline incidence in your population. How that's interpreted is another matter, but starts with something concrete (helps to relate/compare to other risks). Maybe one day @DrTomWaterfield will realise his dream of a consensus on acceptable clinical risk!
