Astute observation. This is a post op localized hemorrhagic effusion behind the left ventricle. The post op state is likely the reason why it’s not circumferential. Looks gnarley

how interesting! Is it localized because the pericardium was violated? Is acute hemorrhage not usually more hyperdense?
