To clarify. "Inform/refus" to me means advising against tPA in elig pt, charting decision to decline and calling off acute prot'col; gutsy but does any1 do it? More insidious is same prior to stroke team who may need to counter ED efforts. Either could trigger an M&M or review.https://twitter.com/jeremyfaust/status/1101988772953108482 …

That’s a helpful clarification. Your feeling of being personally attacked is important for us to know and isn’t the intent. But you have called people unprofessional and threatened disciplinary action for inappropriate reasons. In that context, the source of that becomes relevant

Find reasons to treat patients. There is risk. But stroke is a crappy disease. You guys don't see the patients at 90d, on the rehab ward or long term followup.

Thx for participating in the convo as neurologists. Can we just talk about the data? Your comment seems like a logical fallacy (appeal to emotion). We all agree #stroke is a crappy disease. @reverendofdoubt @jeremyfaust @First10EM @srrezaie @precordialthumhttps://yourlogicalfallacyis.com/appeal-to-emotion …

I don’t get why this spectre of “dubious evidence” keeps coming up re #tPA? There is definitive evidence- pls consider this individ patient metaanalysis https://www.thelancet.com/journals/lancet/article/PIIS0140-6736(14)60584-5/fulltext … or this 3y mortality analysis of IST3 (for those with concerns re survival): https://www.thelancet.com/journals/laneur/article/PIIS1474-4422(16)30139-9/fulltext …

It's a very good question. Why are so many in EM at odds with the rest of the planet on this issue? I have been following this for approx 20 years and I would venture that one very influential voice has convinced many in EM to be skeptical of the data.https://www.youtube.com/watch?v=JyModq-7STM …

IM here- I can’t understand how anyone can look at the data and NOT be skeptical or demand a true quality RCT. Upstairs very few of us have any idea how poor the NINDS trial was we only see the genentech sponsored “are you having a stroke” posters.
