      I write this often. There is pleasantly confused and there is driving the nurses nuts confused and a spectrum in between. If there a simpler more succinct way to convey this?

      Well first of all "confused" doesn't mean much in its own right. Does the person have delirium? dementia? both? Some other cause of cognitive impairment/behavioural disturbance/anxiety/agitation etc

      most of the time the phrase is followed by some sort of descriptor “unknown baseline” or “not at baseline per son”. I just don’t think describing someone as pleasant when you are banging out dozens of notes should be cause for offense.

          describing someone as "pleasant" shouldn't be on either as we shouldnt be making value judgments of that nature. But "pleasantly confused" is misleading, inaccurate, unhelpful. Stick to symtpoms, diagnosis, care needs etc

          Out of curiosity, why write 'pleasantly' at all? Even apart from all of the good advice already given (inaccurate diagnosis, inappropriately reassuring, etc), it takes longer. There might be big efficiency savings to be had in your place, if it's used that much!
