No we made a diagnosis of calcific aortic valve disease with an asymmetric left ventricular remodelling response and associated SAM/LVOTO. We thought the thickening of the mitral valve leaflets might relatE to the SAM. No evidence of mitral stenosis clinically and only v mild MR

Agree definitely possible Mani. Asymmetric patterns of wall thickening fairly common in aortic stenosis, and there wasn't too much LGE so our feeling was probably just that. However unusual to get the SAM and LVOTO!. Guess that's why I thought it was an interesting case!

Thanks Ahmed. Good point. Pre-test probability is quite important here. If the patient were Egyptian I might agree with you, but no much rheumatic heart disease at our centre
