I'm generally of the opinion that Sandy Koufax is a little overrated and was a product of Dodger Stadium. That said, his splits against the Mets and Astros are mesmerizing: 31-4, 1.67 ERA, 11 SHO, 9.6 SO/9.

Don't know how to calculate that, but I'm guessing a 2.57 ERA is maybe a 115 ERA+ for those years.

Although pitchers generally pitch better at home. I expect he would have beaten those stats if he was pitching for a similarly good team in another park. And they are pretty good.
