Holy shit. This op-ed published by @MDMagazine is the nastiest, most misinformed, entitled rant about NPs I can recall seeing in a long time. There's so much wrong here that I don't know where to start. 1/https://twitter.com/mdmagazine/status/998618928958980098 …
In reality, such courses are usually hybrids, like this program at Duke, with an on campus component. 3/ https://nursing.duke.edu/academics/specialized-programs/online-and-distance-learning …

The intent of these courses is to allow working nurses to study to become an NP while still working, like this program. 4/ https://www.ohsu.edu/xd/education/schools/school-of-nursing/programs/masters/advanced-practice-nursing/family-nurse-practitioner/FNP-Distance-Program.cfm …

The reality is that most NPs attend traditional nursing schools and do their clinical rotations at academic medical centers and teaching hospitals and clinics. Most decide to become an NP after working as an RN for several years. 5/

Now, it's true that there are some dodgy online programs out there, but these programs victimize NP aspirants in that their students struggle to find mentors to oversee their clinical rotations and pass the certification exams. 6/

Now what about his rant about "independent" NPs in the 22 states that don't require a collaborative agreement with a physician? 7/

He doesn't seem to realize that NP scope of practice is much more limited than MD scope of practice. NPs cannot practice outside their scope. A pediatric NP can't see adults, for instance. Can't. 8/

In addition to pediatric NPs, there are Family NPs, Adult NPs, Geriatric NPs, Women’s Health Care NPs, Neonatal NPs, Acute Care NPs, Occupational Health NPs, Certified Nurse Midwives, and Certified Registered Nurse Anesthetists. 9/

A confusing patchwork of state laws determines what NPs can any can't do. 10/

I'll stop now, other than to post a summary and discussion I wrote four years ago. 11/https://sciencebasedmedicine.org/expanding-the-scope-of-practice-of-advanced-practice-nurses-does-not-endanger-patients/ …

I'll also conclude with full disclosure. My wife is a pediatric NP. She's also more up to date on her medical knowledge than some of the pediatricians. She would've made an excellent doctor if she had chosen to go that route. 12/

Why
@MDMagazine chose to publish this drivel by a pseudonymous family practice doc, I'll never know. I'm all for strategic pseudonym use online and on blogs (obviously), but I'm much less a fan of them when used in journals and ostensibly respectable magazines. 13/Show this thread -
What is "Raymond Stanz" (yes, that's the pseudonym he's using) afraid of, that Big NP in the form of a giant Stay Puft man will target him for harassment and destruction? He seems not to realize that MDs still have the power. 14/14
