Well, to be devil’s advocate, there was a time that the world being round or landing on the moon seemed “implausible.” In an age of loss of faith in science, valid concerns about peer-review or industry influence or profit motive, scientists need to cultivate more humility.

Humility is extremely important, and I share your concerns about financial COIs in medical science. But that doesn't mean we can't identify characteristics of interventions that make them implausible. Unlike claiming the earth is round, claims that a single intervention 1/2

Agreed, panacea claims are suspect. But CAM is adjunct, not “cure.” Many things do have subtle or system effects: exercise, sleep, stress, sugar intake, etc. Just getting enough sleep or exercising is good for a wide range of medical issues, all without side effects.

Exactly. Exercise is not CAM. Diet is not CAM. Sleep is not CAM. Basically, CAM co-opted those modalities ad a Trojan Horse for quackery like acupuncture, reiki, traditional Chinese medicine, naturopathy, and the like.

Do you find all of those 'therapies' quackery or just some? I've always believed hypnosis was a useful thing, do most studies disagree?

Yet once commercialized, those same previously ridiculed or, even criminalized, therapies becomes accepted. Yes, of course there are approvals, studies, etc. Take quinine. Traditional medication that actually works for malaria. Medical marijuana. Etc.

Medical marijuana is vastly oversold. The evidence for it for pretty much any condition is very weak, other than maybe certain seizure disorders. Certainly smoking it doesn't provide an adequate blood level even for glaucoma.

I have done 2 reviews lately for chronic pain talks and agree with this completely. I am surprised that the physicians who are such strong advocates for efficacy are handing their hats in such weak studies Bias seems clear😢

The endocannabinoid system is one of the most significant neurological discoveries, so recent and still not fully researched, it hasn't been a significant part of the medical curriculum. That is why you are saying these things. "Smoking" -- that's revealing.

It may be a revelation someday that endocannabinoid system may prove significant advance ......no argument there....but just like any other scientific research there has to be verifiable and reproducible proof of efficacy before benefit can be claimed. Not different standard.

I don’t have an opinion on marijuana itself. Generally pediatricians are cautious about the impact on developing brains. My point is that It is something previously criminalized, for which people are sitting in jail even, that now some states are issuing licenses for.
