      Keep justifying 200,000 deaths a year, and M and M's held behind closed doors. Keep denying a truth when I provide proof, and claiming you know what's in episodes you never saw, including ones not aired yet. That's science.

      Indeed it is a reliable number. It's low. Because it doesn't account for the fact medical error is hidden, not reported.       How about this BMJ paper? http://www.bmj.com/content/351/bmj.h3239.long … Same journal 1/10th the rate. The problem of measuring medical error is not simple,or defined by a single estimate - especially not from trigger tools.
