He’s just a doctor who doesn’t want his position and status equated with other “providers”: as in, PAs and NPs.
I guess it's a good way to weed out physicians who are more concerned about their ego being catered to than actually providing healthcare to patients?
Funny story: I took my gran to a doc once + the engraved metal sign outside his door said “Dr. Dr. Smith”. I asked him abt it + he explained that he had an MD *AND* a PhD.
I had to apply cold compresses on my eyes when we got home, that’s how intense an eye-roll he induced. 

His mother must be very very proud of her doctor doctor son!
