Unlike Del's image of people with chronic illness as now paying the price for having lived the good life eating rich food, drinking fine bourbon, smoking cigarettes, etc., it's systemic racism and poverty that result in higher rates of chronic illness among African-Americans..

As for "eating rich food," as Del puts it, the reason so many African-Americans and poor "eat rich food" is because fast food is inexpensive and in neighborhoods that are in food deserts sometimes one of the only convenient sources of food.
