Most hospitals in the US are not-for-profit, but they are expected to function like a business and at the very least not lose money.

I am assuming you are US-based? Your comment struck me as odd. Here in Canada, hospitals get an annual operating budget. They rarely have profit centres in a single-payer, universal medicare system. (Tho, Americans sometimes come here for elective surgeries due to lower costs.)
