Tell me how using tPA as a non-approved Rx for this indication is different. Armstrong discussed it with virtually all the patients. I trust not all pts have delegated PoA, nor do all in NH's have dementia.

Huge gaping legal and ethical differences. tpa is an emergent intervention approved for embolism. He suspected embolism. The other was an experiement. If he did discuss it with "virtually" everybody, why did he administer it to those he didnt have a discussion with?

They both are experimental in the way they were being used. I think in the effort to be advocates for the elderly, most commenting are elder discriminating. Check your sarcasm Kate.

Even *if* we equate both as equally experimental, which I wouldn't necessarily concede, one was an emergent almost a last ditch effort, and one was "let's see what this does! I'll ask if and when its convenient."

Well, again, again, it's a completely legal use (assuming consent) whether you call it "experimental" for the virus or not. It's legal for compassionate use, but it's also legal as off-label.

WITH CONSENT, which wasn't always the case.

The overwhelming majority were able to consent. Armstrong used of the Rx, on those getting sicker. He likely knew that if they went to the hospital, they would have been triaged to the bottom if Tx'd at all. It isn't a study. It was trying to save lives, doing something.

Except for the part about how he himself referred to it as a study.

Yes. He clearly thought he was doing a study, not just off-label prescribing.
