Can a physician organise a study without passing before an ethical commission or enroll patients in a study without informed consent in the USA? Do that in Belgium and you’ll be fired and you’re at serious risk losing your license to practice.

If you believe there is equipoise in the scientific literature about whether or not a drug hurts or helps, and give it to patients without consent/IRB approval in the hopes of finding out whether it hurts or helps, that is extremely unethical and would get you fired.
