      Well, at least I'm not an ophthalmologist. I did do trauma and critical care back in the 1990s. I have experience as a helicopter flight physician. I can intubate, do lines, and, after a refresher, I'm sure I can run ventilators. I'm just rusty.

          I'd hate to miss a chance to thank you, Dr. Gorski. Thank you to you and the whole medical infrastructure, standing ready to help out. The prevailing zeitgeist of scientists, physicians all wanting to contribute is the only redeeming experience of the last few weeks.

          So I think I understand internal medicine. You have to figure out how much fluid is in the body, then you argue with people from different specialties about how much fluid is in the body, then you do an echo, then you add or remove fluid, sometimes both, then discharge.

          Just remember, low stretch, 6-8cc/kg IBW. Covid responds to peep, needing > 12 cm. Keep plateau pressures < 30. Permissive hypercapnea ok if needed to oxygenate. Experiment with modes - not every patient responds the same. Sedate, then paralyze, then prone if needed.
