Yep. Most drugs that appear promising in preclinical experiments fail in clinical trials.https://twitter.com/PeterHotez/status/1240469132518268929 …

Given that one was the HIV protease inhibitor cocktail it always sounded a bit unlikely. If you were speculating you'd have more hope for RNA polymerase inhibitors.

There is a new predicament medicine that is only available in Japan. The nafamostat maybe? The test results are available on the university page of Tokyo University.

if i see another breathless headline about some existing drug that "might" work vs the new #coronavirus, I'm going to become even more irritable than i usually am

If one or more antivirals or treatments are successful, how much of the drug(s) exists? How fast can it be produced? Who gets it? How much does it cost? Is the US going to mandate coverage for it? I doubt it will be so simple.

When I was constant pharmacologist one of the biggest problems I had with pharma clients was that they couldn’t envisage their drug failing. Suggesting plan B, C etc was often considered off message.

Also p-values aren’t everything, and the fact that the confidence interval around this hazard ratio overlapped 1.0 doesn’t mean it failed.

"Mice Lie" is my favie and I know exactly what he means :) Does remind me of an old joke about two mice in a lab, and one says to the other, "I got this scientist so well trained, every time I ring this bell he brings me food."

Really everything before a phase 3 clinical trial is mainly there to weed out the compounds that will *definitely* fail a phase 3 clinical trial.
