Clinical equipoise demands that one group of subjects not be subject to predictable harm by being in a clinical trial. That means you cannot ethically randomize children to a placebo control group in a vaccine trial if there already exists a widely used vaccine for that disease.

Most vaccines are tested against aluminum adjuvants, placebos containing other ingredients or against another vaccine. This is why large set of adverse reactions are never reported during clinical trials, because there is no true placebo control group to test against.
