Or, my personal favorite, out of network physicians. So, the hospital itself is in network, but the doctor providing the service in the ER is not. Or the surgeon who subsequently operates on you during a medical emergency is not. Yay, us.

That is a whole other level of complicated, and I totally agree. If the patients are magically delivered to you simply by walking through the hospital doors, care should be covered. Full Stop

It doesn't need to be that way. Germany's health insurance system is largely private, but there are no out of network hospitals or doctors.
