And if someone feels they could do better they should do it. I’m constantly amazed at the disconnect between people who claim they could treat disease X and their lack of real world responsibility for actually treating disease X.

Doctors in cancer treatment often say that like you. But, you have forgotten to consider about lead time bias and increase in overdiagnosis caused by the spread of cancer screening. That decline cancer mortality ostensibly.
