We are all unconsciously biased in favor of that which favors our interests. The key is being able to understand that, admit it, and always take it into account. Docs unfortunately have an annoying tendency to think they are "above" these considerations. I've seen it in even the best and most caring doctors. It's the same tendency that leads to docs who feed at the pharma trough to claim that the money doesn't affect their judgment.

I can’t remember who but someone said doctors in America want to be paid like businesspeople but treated like do-gooders.

I think it's a blind spot we have. We ARE do-gooders, the vast majority of us. That leads us to have too much faith in our own morality. Too many of us really believe our judgment is not affected by COIs.
