I think the plague of Justinian was worse than any of the medieval and early modern European plagues in terms of the percentage of the population of Western Europe it killed, but it’s now mostly forgotten.

Not to mention smallpox & enteric fevers epidemics that killed 80% of Aztec population in Central America during mid-1500’s. I guess big pharma was doing its dirty tricks during the middle ages at the behest of Moctezuma or Cortes or Charles I....
