We certainly do bilateral prophylactic mastectomies for women who, either through family history or carrying a deleterious mutation that predisposes for cancer, has a lifetime risk of breast cancer above a certain threshold and want the surgery.

This is true for many organs- we can remove the colon and eliminate colon cancer risk (and do so in certain diseases w very high risk). The decision is about far more than just cancer risk. There is surgical mortality (rare) and mobidity (significant) 1/2
