Medical errors are NOT the third leading cause of death in the US. For that to be true, one-third to one-half of all hospital deaths would have to be due to medical errors. I've seen (+ apparently, erroneously cited) that 3rd leading cause of death where it was an aggregation of everything from hospitals, nursing homes, pharmacies, asst living, outpatient clinics and private offices. The miscalculations are based upon various schemes that compound errors in estimates. When actually measured the error rates tend to be around 3% of deaths, and even those measurements are using the “retrospectiscope” of knowing outcome. That would be ~20k deaths/year.
