Avoidable medically? Really? I suppose that my MIL might have survived to be transported back to the care facility if she hadn't had a DNR in place. Would that have counted for these people? Sheesh

Using root cause analysis, almost anything can be deemed preventable. But in many cases it's about as valid as saying the root cause was "First, the Earth cooled..."

I analyzed this data a while back - my god did they screw the stats - bad assumptions - cannot believe it got past the editors. But it continues to be propagated

What editors? It's a report, a white paper. It was never peer-reviewed by a journal, as far as I can tell.
